Brakes Fail
Video is hard to see, but a plane lands on an aircraft carrier. Brakes don't hold after plane is stopped. Plane goes swimming. You get to see them eject first though.

Nice, $2 million plane and it gets wrecked cause someone didn't fix the brakes properly!
